Did you notice the fine print at the bottom? It says the form must be received by Jan 2011. Hmmmm....
ReplyDeleteAnon: Yes, I think what is at the very bottom of the form in the fine print is a misprint because at the TOP of the form on the right - the bullet points next to NOTE - it states:
